Lexington Place Assisted Living provides both basic and memory care assisted
living services to meet the diverse needs of our resident population. Our commitment to each
resident is to create an effective continuum of care with their existing healthcare providers to
preserve independence, personal dignity, and self-respect.
Basic assisted living is a service that offers personal care, meals, housekeeping,
medication management, and social activities for residents who are mostly independent, but
need some assistance with daily living tasks.
Memory care is a service that offers specialized care and support for residents with a
memory care diagnosis who may require additional support and supervision to ensure their
safety and well-being. Memory care services may include dedicated staff with specialized staff
training, individualized care plans, cognitive stimulation programs, social and recreational
opportunities and a secure environment.

Onsite registered nurse for peace of mind
Our registered nurse creates a personalized plan of care for each resident and provides ongoing oversight. It’s important to monitor each individual’s health to identify any changes that occur so we can intervene to reduce the likelihood of illness, injury, or hospitalization.
